We are responsible for processing your personal data. Do you have questions about your privacy? Email or call the owners of the practice using the details above.
What data do we record?
- your name, address, date of birth, phone number and email address;
- your citizen service number (BSN) and your health insurance;
- medical information about your pregnancy, the birth and the postnatal period, and about your baby;
- information about your partner, where needed for your care.
Why?
We use your data to give you good midwifery care, to claim the costs of your care from your health insurer, and because the law requires it. Care providers must keep a medical record (Medical Treatment Contracts Act, WGBO) and use your BSN (Use of Citizen Service Number in Healthcare Act).
Who do we share your data with?
Only with those who need it for your care, or where the law requires it, such as:
- the sonographer, the laboratory and the hospital or gynaecologist;
- your maternity care organisation, your GP and youth health care (the baby clinic);
- the organisations for prenatal and newborn screening, such as the heel prick;
- Perined, the national registry of care around pregnancy and birth (coded, for quality and research);
- your health insurer, for the claim.
Do you object to your data being shared with one of these care providers or organisations? Just let us know, and we will look for a suitable solution together.
We never sell your data and do not use it for advertising.
How long do we keep your data?
We keep your medical record for 20 years after the last change, as the WGBO requires, or longer if needed for good care. Messages via the website or email are kept as long as needed to answer your question or to add your registration to your record. WhatsApp messages are deleted after 1 month.
Security
Your record is kept in a secure system. Only the care providers involved in your care can access it, and only with your permission. The website and forms use a secure connection (https).
The website
- Forms: what you fill in on the contact, registration or intake form is sent to the practice by email and stored in the website’s secure database.
- Cookies: the website does not place cookies. We do not use analytics or advertising cookies.
- Other services: the fonts come from Google Fonts and the reviews from Trustindex. Your browser loads these from their servers, which gives them your IP address. If you click a link to Instagram or Facebook, their privacy policy applies.
Your rights
You may view your data, get a copy and have it corrected. You can also ask for data to be deleted, as far as the law allows. Send your request to info@keita.nl; we will respond within a month.
Do you disagree with how we handle your data? Please tell us first, so we can find a solution together. You can also file a complaint with the Dutch Data Protection Authority.
